LPP is managed as a localised inflammatory skin condition requiring immune suppression at the scalp level. What it actually reflects is a systemic immune dysregulation operating in the context of compromised gut integrity, hormonal imbalance, nutrient deficiencies impairing immune regulation, and environmental triggers, the same upstream terrain driving other autoimmune conditions. Topical and injected corticosteroids, hydroxychloroquine, and other systemic agents reduce the inflammatory attack on the follicle. They do not restore the immune terrain that lost tolerance to the follicle in the first place. Address that terrain and disease activity very often reduces further slowing or halting progression and in some cases achieving sustained remission.
